Medical forms required for Scouting are provided here for your
convenience in downloading. These
are required by the Council and a copy is kept with the Scoutmaster on
all campouts in the event any medical problems are encountered.
Under the Age of 40:
Class I and Class II Medical is a two page form
required for all participants under the age of 40.
Class I occupies page one and part of page two and is a history
form to be filled in by the parents.
Class II occupies the last portion of page two of the form and
is to be filled in by a licensed physician.
The Class I portion (medical history) is required to be updated
every 12 months. The
Class II portion (exam by physician) is required every 36 months.
If your Scout has not submitted a medical form,
you need to download Page 1 and Page 2 of the Class I/II Medical Form
(Scout Medical P-1 and Scout Medical P-2).
Fill in page 1 and have page 2 filled in by a licensed
physician. Then submit the form to the Scoutmaster.

Age 40 or Over:
If you are an adult age 40 or over and you
wish to participate in Scouting activities held at any of the Council
camps, you need to download the Class III Medical Form, pages 1 and 2.
Fill
in Sections I, II, III, IV and VI before visiting your doctor
then have Sections V and VII filled in by a licensed physician.
All this is on page 1 of the Class III Medical Form. Page 2 is
for follow up and, while it must be submitted, you do not need to fill
it out at this time. When all this is done, submit the form to the Scoutmaster.
Class III medical form is required to be updated every 12
months.
