1) Automobile/License/Auto Insurance Information
Required for anyone transporting Scouts on official
activities. BSA insurance covers this travel but only if this
information has been filed. It is necessary to provide this
information only once a year to the troop (or when you change
vehicles/insurance). The information will be listed on the BSA
"Tour Permit" for every activity on which you drive.
If you have not done so this year, please take a moment to print this
form and return it to either the Scoutmaster or Troop Committee
Chairman.
2) Medical Form
Replaces the old class I and class II. This form can be filled
out on your computer and printed out.
The new form is only valid for 12 months.
Required for activities at any BSA activities.

3) Waiver and Authorization for Medical Treatment
Required to be signed yearly by parent or guardian. This
form must be submitted before your Scout will be allowed to
attend any campouts or troop activities. If you have not signed
this in the past year, please take a moment to print the form and
return it to either the Scoutmaster or Troop Committee Chairman.

Troop Demographic Information
Troop 80 uses this to keep up to date with your phone numbers and
E-Mail address. If you have not provided these this year, please
take a moment to print this form and return it to either the
Scoutmaster or Troop Committee Chairman.
5) Merit Badge
Counselor Registration
Circle Ten Council requires that all adult leaders serving as
Merit Badge Counselors register with the Council office. If you
feel you are qualified and wish to teach a particular merit badge,
please print this form and return it to the Scoutmaster or Troop
Committee Chairman. If you are presently a registered Merit
Badge Counselor and wish to teach a merit badge for which you have
not previously registered, please fill out this form.
6) Behavior Agreement
Scout agrees to behave in a Scout-like fashion and abide by the
rules of the troop. Required to be signed yearly by each Scout
and parent.
7) Adult Leader
Required Form to become an adult leader of Boy Scouts.
Make sure that you also sign page 4 of this
application and complete 5 entirely.

Leadership
Application-
Every Scout who is in need of a Troop Level Leadership position needs to
complete this form and turn it in to the SPL.
9)
Parental Informed Consent Agreement for Scouts,
Non-Scout Medical Information sheet
and
Hold-Harmless/Release Agreement.
These forms are necessary for the climbing and
rappelling activities.
10)
Youth Application-
This form is required for joining a Boy Scout
Troop.

2009 Eagle Scout
Application- This form
is required for attaining the Eagle rank.

Eagle Scout Leadership
Service Project Workbook- This is the workbook used for all
Eagle Projects. To start your Eagle Project you need to download
this form and use it for your write-up.
